The following pages are available for anyone to download. They are designed to make sharing important data between teachers and staff as easy as possible for progress-monitoring in today’s classrooms. It’s our way of helping teachers help kids. All we ask is that you don’t remove the copyright information at the bottom. 

Directions:

  1. Type in the student information for your class.
  2. Collect the data indicated.
  3. Most of these sheets are in Microsoft Excel, so take advantage of Excel’s Data–>Sort function. 
  4. Share the files by attaching them to e-mails.

Classroom Data Sheet #1 - (Blackline Master 3-a) Classroom Data Sheet #2 - (Blackline Master 3-b)
This sheet allows teachers to collect and track data in Comprehension, Fluency, and Reading Level for his or her class at three different times during the school year. This sheet allows teachers to collect and track data specific to phonemic awareness and phonics (alphabetic principles). It suggests teachers collect a grade-level appropriate score, a score showing where each student is actually functioning, and a possible phonemic awareness score if students are unable to pass an assessment for even the most basic level of understanding in phonics.
Condensed Data Sheet - (Blackline Master 4-a) Progress Monitoring - (Blackline Master 4-b)
This sheet is designed to condense the four most important scores which are needed for successful differentiation. With Excel’s “Cut and Paste” ability, it is simple to transfer the scores from Blackline Masters 3-a and 3-b, and then use the Data-Sort function to analyze the needs of all the students in just a few clicks of the keyboard. Grouping is much easier with all the information right there in easy view. This sheet allows teachers to collect daily data on each student in all five of the “Big Ideas of Reading” (Phonemic Awarness, Phonics, Fluency, Vocabulary, and Comprehension). It can be used for test scores or anecdotal records.
